When reviewing your insurance coverage with one of our broker-consultants, it is very important that you are well prepared. The following lists outline the important information to have on-hand so we can save you time and make the process as efficient as possible.
Workers' Compensation:
- Legal name of corporation and DBA (if any)
- Federal Employer ID # (FEIN)
- List of all locations
- Description of operations for the company
- Name of President, Secretary and Treasurer if corporation
- Copy of current policy declarations pages.
- The carriers and policy numbers of the previous four annual policies
- Annual estimates of payroll for each classification or job description
- Owners' names and percentages of ownership (stock)
Property & Casualty:
- List of all locations
- Copy of current policy declarations pages for general liability, auto, property, etc.
- The carriers and policy numbers of the previous four annual policies
- Description of operations for the company
- Estimated annual revenue
- Estimated annual payroll excluding clerical and sales employees
- Copy of any company brochure and/or website address
- General Liability requirements (1 Million, 2 Million, etc.)
- Value of business personal property to be covered (furniture, fixtures, TI's)
- Value of EDP equipment to be covered (computers, servers, laptops, etc.)
- Certificate information if required for your business (landlord, contractor, vendor)
- Is office space owned or leased?
- Age, number of stories, square footage and construction type of building
- Square footage of office space occupied
- Central burglar alarm? If yes, name of monitoring company
- Fire sprinkler system?
- Description of other occupants in building (i.e. retail, contractor, warehouse)
- Copy of any current contract requirements for insurance (i.e. leases)
